The Nurse Practitioner (NP) is responsible for the delivery of medical care service to a designated group of INSPIRIS patients working collaboratively with selected! physicians. The NP provides continuing primary care management and is responsible for establishing and maintaining effective relationships with patients, key decision-makers, facility staff, and collaborating physicians. Reports to a Nurse Practitioner Team Leader.

Primary Responsibilities:
Primary Care Delivery
· Delivers cost-effective, quality care to assigned INSPIRIS patients in collaboration with assigned physicians
· Identifies high risk patients and prioritizes care appropriately
· Provides prognosis appropriate preventative care to INSPIRIS patients
· Identifies acute conditions proactively; develops a plan of care and treatment goals that are consistent with the patient’s wishes
· Manages chronic conditions effectively while remaining compliant with INSPIRIS visit requirements
· Performs comprehensive assessment and documents findings in a concise/comprehensive manner that is compliant with OBRA! regulations
· Manages behavioral issues alone or in co! llaboration with physician or specialty provider
· Addresses Advance Planning with all patients in a proactive manner; identifies legal Medical Power of Attorney (MPOA) or surrogate when patient lacks decision making capacity
· Maintains communication with responsible party on a regular basis per INSPIRIS standards
· Manages medications to effectively treat the patient and avoid inappropriate or unnecessary medications
· Facilitates the implementation of the plan of care by engaging the facility staff and ensuring their compliance
· Evaluates the effectiveness, necessity, and efficiency of the plan of care and make revisions as needed
· Utilizes practice guidelines and protocols established by INSPIRIS

Documents in accordance with INSPIRIS standards
Enters / updates patient information in INSPIRIS Enterprise Systems (IES)
Promotes and engages in INSPIRIS’ customer service initiatives

Care Coordinatio! n

Assesses the medical necessity/appropriateness of ancillary services and interfaces directly with the providers and the INSPIRIS case managers
Understands the INSPIRIS benefits and associated policies and procedures, and articulates them effectively to providers, patients, and key decision-makers.
Avoids use of inappropriate, high cost, poor outcome services, by utilizing other resources or benefits
Acts as a resource for the INSPIRIS case managers, providing them with requested information in a timely manner.
Maintains appropriate communication with Team leader and collaborating physicians

Requirements:
Certified nurse practitioner, GNP preferred, Masters required
Long term care experience preferred
Managed care experience highly desirable
Active, unencumbered nursing license
