Job Description
Provides a variety of services to clients and client families. Interacts with families in person and on the phone. Works closely with referring case workers from several referral sources, including Department of Childrenâs Services (DCS), other agencies, residential facilities, attorneys, etc. Utilizes community resources and knowledge thereof to meet the specific needs of each case. Testifies in court regarding case ! work when subpoenaed. Assists in marketing SequelCare of Tenne! sseeâs services to referral sources and individuals.
Knowledge, Skills and Abilities
Requires a minimum of a bachelorâs degree with at least two years of related experience. Demonstrates expertise in a variety of methods of working with families and children. Is proficient in the knowledge of programmatic expectations. Experience with providing in-home services a plus.
Home Office Requirements
All Case Managers work from their own home-office. This requires the following:
Microsoft Word
Faxing capabilities (all paperwork is faxed into our Murfreesboro Office, and then original charts are mailed into the office upon discharge)
Internet Access
Reliable Transportation
Cell phone
Points of Coordination
Supervised and directed by Program Director. Clinical and programmatic supervision provided by the Program Director and Executive Director.
Accountabilities
Me! ets Programmatic Standards
Strategizes with supervisor on how to most effectively work cases
Produces and maintains records of all cases, in accordance with programmatic expectations and policies. Documents sessions/meetings/court in a professional, concise manner noting progress toward service/treatment plans .
Interfaces with the Program Director regarding programmatic and quality assurance issues.
Provides a variety of social services, including: individual, family, and group counseling: communication skills, assertiveness skills, anger management skills, coping skills, depression, anxiety, loss, grief, parenting education, positive discipline, alcohol and drug education with relapse prevention, sexual abuse, physical abuse, domestic violence, and codependency, therapeutic or supervised visitations between foster placed children and birth parents, and transportation of client/siblings/family members as authorized.
Advoc! ate and provide service linking for clients and families
Atten! d court, foster care review board meetings, child and family team meetings, and Individual Education Plan meetings as needed.
Develop service/treatment plans specific to identified client/family needs on a case by case basis using specific, time measurable goals and objectives.
Attends required trainings designated by the Program Director, Operation Director, Executive Director and/or Sequel
Provides on-call, crisis intervention when requested
Maintain strict client confidentiality, safeguard client PHI (protected health information), abide by all corporate policies regarding confidentiality and protection of client data and uphold all HIPAA regulations
