Job Descriptions:
- Responsible for collecting in-depth informa! tion about clients
- Help members plan their objectives and goals
- Conduce face to face assessments in nursing home settings
- Provide telephonic and quarterly face to face assessment
- Additional duties as needed
- Must have an active TN RN licensure in good standing
